Coordinated
Intake for Developmental Services in Halton
- provides a contact point for families /individuals who are interested in accessing residential and non residential supports from the Children's Developmental Services Sector in Halton
Children Services (up to age 18)
- voluntary service
- assist families who have a child with a developmental disability
- information, referral and assistance in accessing services
- provide service coordination
- planning/goal setting
- assist in navigating the service system
- provide information and education on support services within the community
- assist with future planning
- assist in the delivery of the Special Services at Home Program.
Adult Services
(18+)
- must be voluntary client (self referral preferred)
- provide service coordination
- assist in finding accommodation and employment
- provide referral and assistance in accessing community resources and supportive counselling
- assist with future planning
- provide information and education on support services within the community
- planning/goal setting
Respite
Program
- provides short-term parent relief to families/caregivers with
a child/adult who are developmentally and/or physically disabled
and living in the family home
- program provides access to respite both in or out of the family
home
- provides funding to families to assist with the cost of respite
- assists families to locate potential workers for their family
member
- lead agency for the respite

Critical
Service Funds

community crisis funds available to assist families, individuals
and agencies who require short term support to prevent or assist
the individual through a crisis situation, when all other funding
resources have been exhausted
Special
Services at Home program (under 18 years only)

transfer payment agency for the funds approved through the Special
Services at Home program (a program funded by the Ministry
of Community and Social Services/Ministry
of Children and Youth Services), paid to either the worker
or the family

WrapAround
- WrapAround is an innovative process that puts individuals,
families and children with many needs in charge of their lives
- WrapAround gathers family, friends, professionals and/or community
together, providing help and hope by giving families the chance
for self determination, satisfaction and success
Eligibility:
Individuals/families with a child who has a diagnosis of a developmental
disability, multiple needs in three major areas, needs that
are not met through single traditional services, a history of,
or risk of a child being placed outside of the family home
Service
Resolution
- a process facilitated through Halton Support Services for children
and adults with developmental disabilities
- available to families and agencies to present situations involving
individuals whose needs are both complex and urgent and cannot
be met within the existing mandate of available services
Passages
- in partnership with the three local community living associations,
Halton Support Services provides intake and administrative support
to this program
- focus on supporting young people with developmental disabilities
before they graduate from school in planning for the transition
from the school system which may include accessing meaningful
work or daytime activities.
